Christ Community is committed to meeting all the primary care health needs of our patients. We understand the challenge many of our patients face as they work to maintain good health. We strive to deliver quality health care, meeting our patients where they are physically and working with them to gain maintain good health.
Our providers are board-certified Internal Medicine and Family Medicine Physicians. Our staff of Physician Assistant and Nurse Practitioners works closely with physicians to ensure that each patient's needs are addressed. Together, we work as a team also with health coached and counselors to help patients manage the demands of chronic diseases such as High Blood Pressure, Diabetes, Asthma, COPS, Medication-assisted treatment (MAT) for opioid addiction and Heart Disease.
