Rhakevia Carter, FNP-C, is a board-certified Family Nurse Practitioner with a specialization in primary care, prenatal care, and HIV care. She earned her nursing degree from Vanderbilt University School of Nursing and holds board certification from the American Academy of Nurse Practitioners, as well as the Family Nurse Practitioner Certification. Her philosophy of care is centered around treating every patient with the same kindness, dedication, and compassion that she would offer to a family member. She aims to embody the love of Christ in every interaction, ensuring that each individual receives care with dignity and respect.

Paola Plata
Behavioral Health PsyD Intern
Paola Plata, PsyD, is a postdoctoral fellow with the behavioral health team. She earned her PsyD in Clinical Psychology from Azusa Pacific University. With experience in several community counseling centers, Paola completed her pre-doctoral internship at Christ Community Health Services. She specializes in psychological assessments for ADHD and Autism, and offers psychotherapy services in both Spanish and English. Paola is passionate about supporting individuals in the community who are dealing with anxiety, depression, trauma, or everyday stressors. She believes that everyone has intrinsic value and that their story truly matters.

Tara C. Samples
Clinical Psychologist
Tara Samples, Ph.D., LCP-HSP is a Licensed Clinical Psychologist and Health Service Provider (TN) and Nationally Certified Health Service Psychologist. Dr. Samples is the Clinical Psychologist at our Raleigh Clinic, a Behavioral Health Team Lead and the Director of the Behavioral Health Training program at CCHS which includes the doctoral psychology residency, postdoctoral fellowship and masters and doctoral practicum students in psychology, counseling and social work. She has a Ph.D. in Clinical Psychology and an M.A. in Clinical Psychology from Fielding Graduate University, a M.S. in Professional Counseling from Georgia State University and a B.S. in Human Services from Point University (formerly Atlanta Christian College). She completed an APA Internship at the Charles George Veteran’s Administration Hospital with rotations in Primary Care/Health Psychology, PTSD and Substance Abuse. She completed her Postdoctoral work through the Counseling Center at Lincoln Christian University. Dr. Samples has provided behavioral health care for over 20 years, first as a Licensed Professional Counselor and later as Licensed Clinical Psychologist. Prior to joining CCHS she worked as an Assistant Professor of Counseling and Psychology at Lincoln Christian University. Dr. Samples has worked in inpatient and outpatient, private practice and public health settings in Ga, NC, IL and TN. Her clinical interests are in the areas of social justice in healthcare, trauma informed care, health psychology, resilience in the face of trauma and abuse, and the integration of mental health and spirituality. When she is not in the clinic you may find her spending time with her family, supporting the local music and arts community, or gardening.

Tara Seay
Licensed Professional Counselor / Director of Family Solutions
Tara Seay is a native Memphian, born and raised. She attended The Central High school, obtained a B.A. in Criminal Justice with concentration in Pre-law from Lemoyne-Owen College, and M.A. in Mental Health Counseling with specialties in marriage and family, crisis management, substance abuse, and forensic counseling.
She has worked in several counseling environments such as private practice, community outreach and mental health, residential, in-patient psychiatric assessment center, and in home family therapy. She holds certifications in Trauma Focused Cognitive Behavior Therapy, Problem Sexual Behavior, Collaborative Problem Solving, Trauma Informed Care, and Adolescent Community Reinforcement Approach for adolescent substance abuse.
She is currently the Site Director and Family Therapist of the Family Solutions at the Raleigh Christ Community Clinic.

Neelu Sherpa

Dr. Neelu Sherpa is a general dentist with advanced education and experience in dental care. She holds a Master of Biomedical Science (MSBS) and a Doctor of Dental Surgery (DDS) degree. Dr. Sherpa graduated from Touro College of Dental Medicine and completed her residency with NYU Langone and CMDA Dental Residency Plus Program. One of her favorite hobbies includes trying out different cafes and food places across various countries and cities, which not only enriches her interests but also fuels her creativity and appreciation for diverse cultures and cuisines.

Elise Shockley

Dr. Elise Shockley graduated from the UNC Adams School of Dentistry in 2014. She then completed the Advanced Education in General Dentistry (AEGD) program at NYU Langone and UTHSC in 2015, followed by the CMDA dental residency’s Plus Program in 2017. Throughout her career, she has held various positions at Christ Community Health Services, including Dentist, Lead Dentist, and Dental Residency Director. Elise is married to her husband, Phillip, and together they have three children. As a family, they enjoy exploring the great outdoors. A lover of travel, they immerse themselves in diverse cultures, gaining unique experiences and broadening their perspectives. One of their greatest joys is exploring different cultural cuisines, savoring new flavors, and appreciating culinary traditions from around the world. This passion for food and culture enriches their holistic approach to patient care, fostering empathy and a global perspective.

Kimberly Wilson is a board-certified Family Nurse Practitioner by the certifying body of the American Academy of Nurse Practitioners. Kimberly received her Master of Science in Nursing specializing in family nurse practitioner (FNP) from Arkansas State University in Jonesboro, Arkansas. Kimberly has experience as an FNP in primary care and family planning after years of providing care as a registered nurse in adult and pediatric oncology and med-surgical/orthopedics hospital settings. Kimberly is a God-fearing nurse practitioner with a strong desire to give back to the community and has a passion for teaching and providing holistic care, not only for physical ailments but also for the mental, emotional, and social aspects of the patient's well-being, to promote overall health and quality of life for patients.
